BreakThrough Physical Therapy is seeking a compassionate and skilled Physical Therapist to join our Healthcare & Medical Services team. As a Physical Therapist, you will deliver evidence-based, patient-centered care to individuals with a wide range of musculoskeletal, neurological, and post-surgical conditions. You will perform comprehensive evaluations, develop individualized plans of care, and implement goal-oriented treatment programs to improve mobility, function, and pain relief. Working within a collaborative, interdisciplinary environment, you will use manual therapy, therapeutic exercise, gait and balance training, neuromuscular re-education, and appropriate modalities to help patients achieve meaningful, measurable outcomes. In this role, you will manage a diverse outpatient caseload, document thoroughly and accurately in our electronic medical record system, and regularly reassess and adjust treatment plans based on progress and current best practices. You will educate patients and their families on diagnosis, treatment plans, home exercise programs, and strategies for long-term self-management and injury prevention. You will actively contribute to case discussions, coordinate care with referring providers and other healthcare professionals, and participate in quality and performance improvement initiatives.
