Your role will be crucial in the development and enhancement of our Palliative Care and Hospice Services, primarily within the inpatient hospital environment, where you will round on various units, including the inpatient palliative care unit. Additionally, you will play a significant role in the success of our Internal Medicine Graduate Medical Education (GME) program.

Primary Responsibilities:

  • Conduct inpatient palliative care consultations for patients facing advanced or complex health issues.
  • Address pain management and alleviate distressing symptoms such as dyspnea, nausea, anxiety, and delirium.
  • Support the management of end-of-life symptoms within the inpatient palliative unit.
  • Facilitate discussions on advanced care planning, including goals of care, advance directives, and code status.
  • Work collaboratively with an interdisciplinary team, including nurses, social workers, chaplains, and case managers.
  • Coordinate care with primary teams, specialists, and case managers.
  • Assist in transitions of care across different settings.
  • Document all patient care and decisions in the electronic medical record in line with organizational standards.
  • Educate patients, families, and hospital staff on palliative care principles and available options.
  • Engage in team meetings, quality improvement projects, and educational activities.
  • Participate in the training of Internal Medicine and Family Medicine residents, coordinating rotations with 2nd and 3rd-year residents.
